CAN I GET A SAME-DAY APPOINTMENT?

If you need an urgent visit, please call the office directly and we will do our best to arrange an appointment.  In some cases, a phone visit, or other electronic visit may be offered.  Remember that your doctor is on call for you after hours if you have an urgent issue.

HOW DO I GET MEDICAL RECORDS TO/FROM COSMAS PRIMARY CARE?

Please fill out the Release of Medical Records form linked here and mail or fax it to the doctor's office you are getting the records from.

WHAT ARE YOUR HOURS?

Currently, our office is open:
Wednesday | 9 am - 5 pm
Friday | 12 pm - 5 pm
Appointments available for urgent care as needed outside of these hours at the discretion of the doctor.

DO YOU HAVE A PATIENT PORTAL?

Yes!  You can look at your problem list, medications, care plans, referrals, lab results, on our patient portal.  If you do not have a confirmation code, please email us to get one. Login here:
